ADA Memo

As part of the joint initiative between ADA and Agency for Integrated Care (AIC), Support for Persons living with dementia Over the COVID-19 period or SPOC-19, persons with dementia and their caregivers can show any one of three identifiers to enforcement authorities and help them to offer appropriate response and assistance.

Application for the ADA Memo

Fill up the electronic form below to receive an ADA Memo that will identify the holder to be a person living with dementia. Do note that screening via phone will be conducted to ensure that the application is verified and authentic. Upon approval, the ADA Memo will be processed in 2 working days. This memo is applicable for all Singapore Citizens, Permanent Residents as well as foreigners living/working in Singapore.

  • Please enter a valid email address if you wish to receive a copy of your application details. This does not have to be the email address of the Person with Dementia.

  • For example, if the NRIC number is S7654321B, enter "321B"

The ADA Memo is the newest identifier, introduced to complement two existing supporting documents, the Safe Return Card and the ICED Sticker (In Case of Emergency, Dial). These identifiers are part of the SPOC-19 initiative.
